We are seeking a skilled and motivated Speech-Language Pathologist (SLP) to join our rehab team on a PRN basis . Your expertise will play a key role in improving communication, cognition, and swallowing function for our residents.

$45-$50/hour Flexible PRN scheduling Reliable hours available Opportunities across our South Carolina facility network Supportive, interdisciplinary team environment Position Summary As a Speech-Language Pathologist (SLP) , you will evaluate and treat residents with speech, language, cognitive, and swallowing disorders. You will collaborate closely with physicians, nursing staff, and therapy team members to create individualized care plans and improve patient outcomes. Key Responsibilities Evaluate residents within 24 hours (or 1 business day) of physician referral Develop and implement individualized treatment plans with physician approval Provide therapeutic interventions based on patient needs and goals Monitor patient progress and adjust treatment plans accordingly Maintain accurate and timely clinical documentation Participate in care conferences, discharge planning, and interdisciplinary meetings Educate staff and caregivers on therapy techniques and maintenance programs Supervise Speech-Language Pathologist Assistants (SLPAs) as applicable Conduct in-service training for facility staff Qualifications Master's or Doctoral degree in Speech-Language Pathology Active, unencumbered SLP license in South Carolina (required) Certificate of Clinical Competence (CCC-SLP) preferred Experience in a skilled nursing or rehab setting preferred Strong knowledge of speech, language, cognitive, and swallowing disorders Familiarity with PointClickCare (PCC) and Casamba preferred Excellent communication and interpersonal skills Ability to work independently and as part of a team Why Join Chandler Creek Post Acute?
