Position Summary Maple Springs Skilled Nursing & Rehabilitation is seeking a compassionate, knowledgeable, and dedicated Part-Time Speech-Language Pathologist (SLP) to join our rehabilitation team. The SLP will provide skilled speech, language, cognitive-communication, and swallowing services to residents in a skilled nursing environment. The successful candidate will work collaboratively with residents, families, caregivers, nursing staff, and the interdisciplinary rehabilitation team to promote safety, functional independence, and quality of life. Essential Responsibilities Complete comprehensive speech, language, cognitive-communication, and swallowing evaluations as indicated. Develop individualized treatment plans based on resident needs, functional goals, and physician orders. Provide skilled speech-language therapy services in accordance with established treatment plans. Evaluate and treat swallowing and feeding difficulties and make appropriate recommendations to promote safe oral intake. Provide cognitive-communication treatment addressing areas such as memory, attention, problem-solving, orientation, executive functioning, and safety awareness. Address speech and language deficits affecting communication and participation in daily activities. Recommend appropriate compensatory strategies, therapeutic techniques, and adaptive equipment as clinically indicated. Provide education and training to residents, families, caregivers, and facility staff regarding communication, cognition, swallowing safety, and carryover strategies. Monitor resident progress and modify treatment plans as clinically appropriate. Communicate changes in resident status, treatment response, and recommendations to the appropriate members of the healthcare team. Complete accurate and timely evaluations, daily notes, progress reports, recertifications, discharge summaries, and other required documentation. Participate in interdisciplinary care planning and collaborate with nursing, dietary, therapy, physicians, social services, and other healthcare professionals. Assist with discharge planning and provide recommendations to support safe transitions of care. Maintain resident dignity, privacy, safety, and confidentiality at all times. Follow facility policies, professional standards, and applicable federal and state regulations. Qualifications Master's degree in Speech-Language Pathology from an accredited program. Current Speech-Language Pathologist license in the State of Utah or eligibility for Utah licensure. Current Certificate of Clinical Competence (CCC) through ASHA, or appropriate clinical fellowship/eligibility status. Current CPR/BLS certification, or ability to obtain as required. Experience in skilled nursing, geriatrics, rehabilitation, or long-term care is preferred but not required. Strong clinical reasoning, communication, organization, and documentation skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Compassionate and professional approach to resident care.
