Role:

Home health Speech Language Pathologist (SLP) performing evaluations and therapeutic interventions for adult and geriatric patients in clients' homes.

Responsibilities:

Conduct home visits within designated areas. Perform assessments to diagnose speech, language, communication, and swallowing issues. Develop, implement, and revise personalized treatment plans based on evaluations, physician's orders, and client goals. Provide rehabilitative care and patient, family, or caregiver education to promote health, safety, and independence. Accurately document observations, interventions, and evaluations using electronic medical records.

Qualifications & Skills:

Current Vermont Speech Language Pathologist license. Certification from the American Speech and Hearing Association or equivalent educational and work experience. Experience with electronic medical records preferred. Strong communication, assessment, and documentation skills.
