You will deliver individualized therapy and collaborate with an interdisciplinary team to improve communication, cognition, and swallowing. The position offers flexible scheduling within a dedicated service area.

Key Responsibilities:

Provide skilled speech-language pathology services to patients in their homes Evaluate patients with speech, language, communication, cognitive, and swallowing disorders Develop individualized treatment plans based on patient needs, diagnoses, and functional goals Deliver therapeutic interventions to improve communication, cognition, and swallowing function Treat patients with a variety of neurological, medical, and age-related conditions Educate patients, family members, and caregivers about treatment plans, compensatory strategies, and home exercise programs Recommend appropriate communication strategies and adaptive equipment when clinically indicated Monitor patient progress and adjust treatment plans as needed Complete accurate and timely clinical documentation in accordance with home-health standards Collaborate with physicians, nurses, occupational therapists, physical therapists, social workers, and other interdisciplinary team members Participate in care coordination and communicate changes in patient condition Maintain a professional and compassionate approach while working independently in patients' homes

Qualifications:

Master's degree in Speech-Language Pathology from an accredited program Current Idaho Speech-Language Pathology license or eligibility for Idaho licensure Certificate of Clinical Competence from ASHA (required per licensing standards) Valid driver's license and reliable transportation Ability to travel to patient homes throughout the assigned service area Strong clinical assessment, documentation, communication, and organizational skills

Preferred Skills:

Experience providing speech therapy in a home health setting Experience treating adult and/or geriatric patients Knowledge of dysphagia and swallowing disorders Experience with cognitive-communication disorders Experience with neurological conditions and rehabilitation Strong patient and caregiver education skills Ability to work independently while maintaining communication with interdisciplinary care team Excellent time management and scheduling skills Commitment to patient-centered care and functional outcomes
