Setting:

Skilled Nursing Facility Responsibilities Conduct comprehensive speech-language pathology evaluations for residents in an acute care-oriented skilled nursing setting. Develop, implement, and monitor individualized treatment plans addressing communication disorders and dysphagia. Deliver therapy sessions, document progress, and adjust plans of care based on patient response. Collaborate with an interdisciplinary team (nurses, therapists, physicians) to optimize patient outcomes and ensure continuity of care. Provide patient and family education on diagnoses, progress, and home exercise programs; ensure safety and infection-control standards. Maintain accurate documentation in charting systems and comply with facility policies and applicable regulations.
