We are seeking a Per-Diem Home Care Aide to provide high-quality, one-on-one care in patient homes. In this \"as-needed\" role, you will be the heart of our service, building genuine relationships while following personalized care plans. You are the ideal candidate if you are highly organized, detail-oriented, and possess the strong communication skills necessary to provide top-tier care on a flexible basis. Prior Home Health or Hospice experience is a significant plus.
Responsibilities Personalized Care:
Assist patients with daily activities (ADLs), including mobility, bathing, and transfers (bed, wheelchair, or automobile).
Home Support:
Maintain a healthy environment by changing linens, performing light laundry, and assisting with general home cleanliness.
Clinical Reporting:
Maintain accurate records of patient care, condition, and progress; communicate changes in status directly to supervisors.
Mental & Emotional Well-being:
Build meaningful connections through conversation, reading aloud, and companionship.
Health Maintenance:
Assist with medication reminders/administration and immunizations as directed.
Engagement:
Lead patients through prescribed exercises and daily activities to support physical and mental health.
Qualifications Education:
High School Diploma or GED.
Experience:
Minimum of one year of prior professional caregiving experience.
Licensing:
Valid Driver's License and reliable transportation are required.
Mindset:
Ability to work independently and adapt to different home environments on an as-needed basis.
