Job Description

We are looking for a Home Care Coordinator for office in Long Island City in Queens. Experience with HHA Exchange Software preferred. This is a full time position
Responsibility:
  • Bilingual
  • (English/Spanish)
  • Staff cases to provide services for patients.
  • Clerical work
  • Assigning/scheduling Home Health Aids to provide services to patients
  • Answering phones for the agency
  • Keeps an on-call log
  • Coordinates PCA and RN services.
  • Prepares and sends employee profile
  • Ensure aides clock in and out and input tasks and time sheets according to POC
  • Other duties as assigned by the Administrator First 3 months is a probation period
