Intake Coordinator - Home Health Agency Join Our Growing Team! We are seeking a detail-oriented and compassionate Intake Coordinator to join our busy home health office. This role is essential in ensuring new patients are smoothly admitted and receive timely, high-quality care.

Responsibilities:

Receive and process new patient referrals Verify insurance eligibility and obtain authorizations Coordinate start-of-care services with clinical staff Communicate with hospitals, physicians, case managers, and families Maintain accurate patient records and documentation Ensure compliance with home health regulations and company policies Provide excellent customer service by phone and email

Qualifications:

Previous intake, scheduling, or admissions experience preferred Home health or healthcare office experience strongly preferred Knowledge of Medicare and insurance verification a plus Strong communication and multitasking skills Proficient in computer systems and EMR software Organized, dependable, and team-oriented
