Our clinicians focus on what matters most — helping patients maintain mobility, independence, and overall quality of life where they live.
We're currently seeking motivated Physical Therapists who want the flexibility, autonomy, and meaningful patient relationships that come with home-based care.

With multiple homecare coverage areas available, we work with you to build a caseload in locations that fit your schedule and preferences.

Responsibilities:
Accept and treat patients within your designated geographic area Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plansDevelop a plan of care and treatment goals in consultation with the physician, patient, and familyPlan, prepare and implement customized therapy programs to restore function and promote independence Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniquesProvide timely and accurate documentation for each visitCommunicate effectively with patients, families, and interdisciplinary team members, especially around discharge planningDeliver care in compliance with agency policies and professional practice standardsQualificationsPhysical Therapist Program GraduateLicensed Physical Therapist in the state of New York or eligible.Home health experience is preferred but not required
