Home Health Aide

Home Health Aide (HHA) - Job Description We are seeking a compassionate and reliable Home Health Aide (HHA) to join our team at The Roads Home Health Care. The HHA will provide personal care and support services to patients in their homes, assisting with activities of daily living while promoting independence, comfort, and dignity. Responsibilities Provide assistance with personal care, including bathing, grooming, dressing, toileting, and mobility. Assist with meal preparation, feeding, and maintaining proper nutrition. Perform light housekeeping tasks such as laundry, changing linens, and maintaining a clean and safe environment. Monitor and report changes in the patient's condition to the supervising nurse or care coordinator. Assist with medication reminders (as permitted by state regulations). Provide companionship, emotional support, and encouragement to patients. Follow the patient's individualized care plan as directed by the nurse or supervisor. Ensure compliance with all agency policies, state, and federal guidelines. Requirements High School diploma or equivalent (preferred). Current and valid Home Health Aide (HHA) certification (per state requirements). CPR certification (preferred or required by state regulations). Prior experience in home health, nursing home, or hospital setting (preferred but not always required). Ability to follow instructions, care plans, and maintain accurate documentation. Reliable transportation and willingness to travel to patient homes. Strong interpersonal skills, compassion, and patience. Must pass a background check and health screening (TB test, physical, drug screen as applicable). Work Environment Patient homes across designated service areas. Flexible scheduling available (days, evenings, weekends). Supportive team environment with opportunities for growth.
