This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia care. Whether assisting with daily activities or managing medical needs, your efforts will foster a safe, respectful, and nurturing environment. Duties Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools such as Hoyer lifts or other assistive devices. Monitor patient health by observing vital signs, documenting medical information accurately in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly. Support medication administration following prescribed schedules while adhering to HIPAA regulations to ensure patient confidentiality. Assist with specialized care needs such as catheter care, hospice support, Alzheimer's or dementia care, and memory care strategies. Conduct patient observation to detect any signs of health deterioration or safety concerns; respond swiftly and appropriately to emergencies. Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation to promote overall wellness. Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment that encourages independence and comfort. Utilize medical terminology knowledge when updating medical records or communicating with healthcare teams. Operate EMR (Electronic Medical Records) or EHR systems efficiently for accurate documentation of patient care activities. Provide companionship and emotional support to enhance mental well-being while respecting patient privacy and dignity. Support clients in community settings through home & community care services that foster social engagement and quality of life. Follow all safety protocols during patient transfers using equipment such as Hoyer lifts or other mobility aids; ensure proper use at all times. Maintain compliance with all healthcare regulations including HIPAA standards; participate in ongoing training on CPR, first aid, and other certifications as required. Qualifications Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments. Strong knowledge of patient observation techniques, vital signs measurement, medication administration, and medical documentation procedures. Familiarity with working with people with developmental disabilities, Alzheimer's disease, dementia, or memory impairments. Experience operating EMR/EHR systems for accurate record keeping; understanding of medical terminology is preferred. Valid CPR certification; additional certifications such as hospice care training or catheter care are advantageous. Ability to assist with ADLs effectively while demonstrating compassion and patience. Excellent communication skills to collaborate with healthcare teams and family members effectively. Ability to work independently in clients' homes while maintaining high standards of safety and professionalism. Physical stamina to perform caregiving duties that include lifting, transferring patients safely using Hoyer lifts or similar equipment, standing for extended periods, and performing light housekeeping tasks. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Prior experience working with seniors or individuals with disabilities is highly desirable; home & community care experience is a plus.
