Pay Rate:

Job Overview We are seeking compassionate and dedicated Home Health Aides (HHA) and Personal Care Aides (PCA) to provide essential in-home support to individuals requiring assistance with daily activities. The ideal candidates will possess a strong commitment to patient-centered care, excellent communication skills, and a thorough understanding of medical and personal care procedures. This role offers an opportunity to make a meaningful difference in the lives of clients by delivering respectful, personalized care within their home environments. Job Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and observe patients' health status, and reporting any changes to healthcare professionals promptly. Utilize Electronic Medical Record (EMR) systems to document care activities, patient progress, and medical records securely in compliance with HIPAA regulations. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques. Assist with light housekeeping tasks including cleaning, laundry, and meal preparation to promote a safe and comfortable environment. Collaborate with healthcare teams to ensure comprehensive home and community care tailored to individual needs. Support hospice or palliative care clients with compassionate attention to comfort and dignity. Qualifications Prior experience in home health care, assisted living facilities, nursing homes, or long-term care environments is highly desirable. Knowledge of medical terminology and familiarity with medical documentation processes. Strong observational skills for patient monitoring and the ability to recognize urgent health issues promptly. Experience operating EMR systems for accurate record keeping while maintaining HIPAA compliance. Excellent communication skills combined with a respectful approach toward diverse populations of seniors or individuals with special needs.
