Role Highlights:

Provide personalized healthcare services to geriatric patients in their residences. Conduct comprehensive clinical assessments, diagnoses, and develop tailored treatment plans. Collaborate with interdisciplinary teams for holistic patient care. Educate patients and families on disease management and lifestyle adjustments. Build trusting relationships with patients, serving as their advocate and source of empathy.

Qualifications:

Valid Nurse Practitioner license. Experience or strong interest in geriatric care. Excellent communication and interpersonal skills.
