Full Job Description Job Title:

Home Health RN Assignment Length:

17 weeks (potential to Perm)

Location:

Detroit, MI Shift:

8 am-4:30 pm; and one Saturday every

Month Pay Rate:

$54/hour

Responsibilities:

Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Perform admission processes and case management responsibilities in accordance with the Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Experience:

1 year Certified Home Care experience

• a Must!! 1 year Case Management Experience
• a Must!! Ability to travel to cases as assigned
• a Must!

Job Requirements:

RN License in MI BLS Certification Must have a valid driving license and a reliable means of transportation to commute. Who We Are?
