Care Center RN 7:00 AM - 7:30 PM 1 Contingent opening (24 hour minimum monthly requirement) 7:00 PM - 7:30 AM ( $5 shift diff increase for nights ) 1 Part Time opening (24 hours weekly) 2 Contingent openings (24 hour minimum monthly requirement)

Waterford Care Center Reports to: Director of Angela Hospice at Waterford The Angela Hospice Care Center at Lourdes is a 15 bed hospice inpatient unit. More than a job. Live your purpose. We're looking for a couple Care Center RN's who share this vision.
Primary Duties and Responsibilities:
Completes an ongoing comprehensive assessment of patient and family to determine hospice needs Provides quality, professional nursing care to patients utilizing specialized nursing skills related to end-of-life care May initiate the patient's plan of care and includes the patient and the family in the planning process Performs comprehensive assessment of assigned patients in a timely manner upon admission and as required Assesses and evaluates patients and makes necessary revisions to the plan of care as patient status and needs change Initiates appropriate preventive and rehabilitative nursing procedures Administers and documents medications and treatments as prescribed by the physician Orders medications as needed from pharmacy Educates the patients and families regarding medications, treatments and procedures, disease process, and end of life care/process Uses established confidential communication channels to communicate pertinent information Communicates patient/family concerns to appropriate interdisciplinary team member and initiates a corrective action if required Assists the patient with activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care Lifts, moves, and transports patients using proper body mechanics or lifting devices Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given Records pain/symptom management changes/outcomes as appropriate Communicates with the physician and/or nurse practitioner regarding the patient's needs and reports changes in the patient's condition Communicates and collaborates with community health related persons to coordinate the care plan Teaches the patient and family/caregiver self-care techniques as appropriate Provides and maintains a safe environment for the patient Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the physical, emotional, and spiritual needs of the patient and family/caregiver Attends interdisciplinary group meetings as needed Ensures that arrangements for equipment and other necessary items and services are available Maintains the comfort, privacy, and dignity of each patient and interacts with patients in a manner of compassion and empathy Reports an incident or evidence of abuse or violation of residents rights to the Lead Nurse, Director of Inpatient Services, or any administrative staff Provides clinical supervision of LPNs, SNAs, hospice aides, and volunteers and delegates responsibilities when required
Qualifications:
Graduate of an accredited school of nursing Current license in the State of Michigan CPR Certified Hospice experience in an inpatient or home care setting preferred Excellent organization and time management skills Proficient computer knowledge and skill Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist Ability to work with little or no supervision Capacity to lift, position and/or transfer patients Auditory and visual skills Proficient interpersonal relations and communicative skills Knowledge of Federal/State, NHPCO, and CHAP regulations and guidelines
Pay:
$36.00 per hour
