Job Description

Overview Join our compassionate hospice team as a Certified Nursing Assistant (CNA) in Crosby, Texas, and make a meaningful difference in the lives of patients and their families during their most vulnerable moments. As a vital member of our care team, you will provide personalized, respectful, and attentive care to individuals receiving end-of-life services. This role offers an opportunity to deliver comfort, dignity, and support through hands-on assistance and compassionate presence. Duties Assist patients with activities of daily living such as bathing, grooming, dressing, and mobility support using equipment like Hoyer lifts when necessary Monitor and record vital signs, patient observations, and changes in condition accurately within electronic health record (EHR) systems such as Epic or Cerner Support medication administration under supervision, ensuring adherence to HIPAA regulations and safety protocols Provide emotional support and companionship to patients experiencing dementia or Alzheimer's care needs, including memory care strategies Collaborate with interdisciplinary teams to develop and implement individualized care plans for hospice patients Maintain infection control standards through aseptic technique and proper medical documentation Assist with medical procedures such as catheter care, phlebotomy, and basic life support during emergencies Join us in delivering exceptional hospice care that emphasizes dignity, comfort, and respect.
