Job Description

Position Summary The Hospice RN Case Manager provides compassionate, high-quality care to patients and families facing terminal illness. This role coordinates all aspects of the hospice patient's care, ensuring comfort, dignity, and emotional support in accordance with the hospice philosophy and regulatory standards. The RN Case Manager serves as the primary point of contact for the patient, family, and interdisciplinary team. Key Responsibilities Patient Care Coordination Conduct comprehensive initial and ongoing assessments of patients' physical, emotional, and spiritual needs. Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team (physicians, social workers, chaplains, aides, and volunteers). Provide skilled nursing care, including pain and symptom management, medication administration, and wound care. Communication & Collaboration Serve as the primary liaison between the patient, family, and hospice team. Educate patients and families on disease progression, medication usage, and the dying process with sensitivity and clarity. Coordinate with physicians and other healthcare providers to ensure continuity and quality of care. Documentation & Compliance Maintain accurate, timely, and detailed clinical documentation in accordance with agency policies and regulatory requirements. Ensure compliance with Medicare/Medicaid hospice regulations and accreditation standards. On-Call & Emergency Response Participate in on-call rotation as needed, providing after-hours support and visits to manage urgent patient needs. Provide crisis intervention and emotional support during end-of-life events. Qualifications Current Registered Nurse (RN) license in Texas. Minimum 2 years of clinical nursing experience (hospice, palliative care, or home health preferred). Current CPR certification. Valid driver's license, auto insurance, and reliable transportation. Excellent communication, organizational, and critical-thinking skills. Empathy, emotional resilience, and a strong commitment to patient-centered care. Work Environment This position involves travel to patients' homes, assisted living facilities, or nursing homes. Main areas are Bay City and Palacios.
