Job Description

Job Summary We are seeking an experienced and compassionate On‑Call Registered Nurse to provide skilled nursing care and urgent clinical support outside of standard business hours. As a key member of the interdisciplinary team, the On‑Call RN ensures continuity of care by responding to patient and caregiver needs during evenings, nights, weekends, and holidays. This role requires strong assessment skills, the ability to work independently, and a commitment to hospice philosophy by delivering timely symptom management, education, and emotional support to patients and families in their homes or care facilities. Monday - Thursday 5pm to 8am Friday 5pm to Monday 8am
Salary:
RN PPV- $5/hour when on call, $75 routine visit, $125 death visit, $225 admit and $40 for Training/Meeting ect.. This position is currently part-time due to a census under 50, with the opportunity to transition to a salaried role as patient volume increases. Essential Duties & Responsibilities On‑Call Patient Care Respond promptly to after‑hours patient calls to assess needs and provide clinical guidance via telephone or in‑person visits as indicated. Perform focused and comprehensive nursing assessments to address changes in patient condition and assist with after hour admissions. Provide skilled nursing care including pain and symptom management in accordance with physician orders and hospice standards. Administer medications and treatments as prescribed and outlined in the plan of care. Initiate and revise care plans based on patient condition and on‑call assessments. Assist patients with activities of daily living as needed to promote comfort and dignity. Perform medication reconciliation as required in coordination with the interdisciplinary team. Ensure arrangements for medications, DME, supplies, and services are in place during on‑call hours. Maintain accurate, timely clinical documentation in the electronic medical record (EMR) in compliance with organizational policies and regulatory requirements. Communication & Coordination Communicate changes in patient condition to the Hospice Medical Director, attending physician, and interdisciplinary team. Obtain and implement physician orders as required. Provide education, support, and reassurance to patients and caregivers during after‑hours situations. Collaborate with hospice aides and other team members to ensure continuity of care. Report incidents, concerns, or complaints to the Director of Patient Services or Administrator as appropriate. Review non‑covered items and provide required notices (including NOMNC) in accordance with state and federal regulations. On‑Call Schedule Expectations While on call, remain reachable by phone and able to respond promptly to patient needs. Be prepared to provide telephonic triage or make in‑person patient visits as clinically indicated. On‑call schedules may be adjusted with advance notice to ensure patient coverage. Qualifications Education & Licensure Graduate of a National League of Nursing (NLN) accredited school of nursing. Current Registered Nurse (RN) license in the state of practice. Current CPR certification. Valid driver's license with reliable transportation and active auto insurance. Experience Minimum of one (1) year of professional nursing experience within the past three (3) years OR Bachelor of Science in Nursing (BSN) with current RN licensure. Hospice, home health, community health, or on‑call/triage nursing experience preferred. Skills & Abilities Strong assessment, clinical judgment, and critical‑thinking skills. Excellent verbal and written communication skills. Ability to work independently with minimal supervision. Comfortable managing urgent clinical situations. Physical ability to lift, reposition, and transfer patients; frequent standing, walking, bending, and driving required. Why Join Us?
