Hospital Liaison

Are you a relationship-driven marketer who thrives on building trust and turning connections into consistent referrals? Pleasant Meadows Senior Living , a well-established Skilled Nursing Facility, is seeking a Hospital Liaison to lead our outside marketing and referral development efforts. This is a highly visible, field-based role where you'll build strong relationships with hospital discharge planners, case managers, social workers, and community partners — serving as the key link between acute care and our skilled nursing team. What You'll Do:

Build and maintain strong referral relationships with hospitals and community partners Manage and grow a defined referral territory Identify gaps in referral patterns and implement strategies to increase census Educate partners on our clinical capabilities and admissions process Collaborate with admissions and leadership to convert referrals into placements Track outreach activity, referral trends, and growth metrics

What We're Looking For:

Experience in SNF marketing, hospital liaison, admissions, or post-acute sales Proven ability to grow referrals and drive census Strong follow-up and relationship management skills Knowledge of Medicare, managed care, and discharge workflows Self-motivated, strategic, and comfortable working in the field
