Job Description

Hospital Pharmacist PRN Mercy Health Corporation $119,940.61 - $179,910.92 United States, Illinois, Rockford May 15, 2026 ESSENTIAL
DUTIES AND RESPONSIBILITIES
Prepares, dispenses medication orders per physician request according to established policies, procedures, & protocols. Provides accurate, adequate, and timely drug information to professional staff, patients and patient families. Documents these activities, as needed. Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols. Detects and accurately reports, suspected adverse drug reactions and medication occurrences. Assists other professional staff to identify med errors/ adverse reactions. Participates in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; and maintains logs/records and other documentation, as assigned. Assists with pharmacokinetics consult service/drug dosing per organizational protocol. Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes. Assists professional staff with managing patient's pain. Supervises, directs, and coordinates the daily activities of the pharmacy technicians/ support personnel. Provides input for pharmacy support personnel performance appraisals. Performs essential duties of the pharmacy director in his/her absence. Adheres to health system policies and procedures. Complies with all requirements related to risk management, safety, security, fire, and infection control. Complies with all applicable federal, state, and local laws, rules, and regulations. Actively participates in the department's quality improvement activities, hospital committees, department/team meetings. Assists new hires in orientation, education, and training programs. Reviews literature and other materials, as assigned. Maintains competence required for current job title/position including: maintains current professional licensure and completes all competence/skills assessment requirements. Maintains formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug selection. Monitors any investigational drug Issues controlled substances to patient care areas & maintains records required by law. Resolve discrepancies as needed. Maintains professionalism and conducts self as a role model, serving as a resource for the other retail pharmacy partners. Maintains practice by ongoing professional literature review, continuing education and self-evaluation.
EDUCATION AND/OR EXPERIENCE
Graduate of an accredited Pharmacy program
CERTIFICATION/LICENSURE
Wisconsin and/or Illinois license required
OTHER SKILLS AND ABILITIES
Computer experience, including internet and email. Strong accuracy skills Excellent interpretive/analytical skills for pharmaceutical calculations for drug dosage, IV rates, etc
LEVEL OF SUPERVISION N/A SUPERVISES
This position supervises technicians. This position is involved in the recruitment and orientation process and provides performance feedback and input for performance evaluation purposes. May implement coaching, counseling, and corrective action when needed.
