Care Concierge & Thoughtful Engagement Specialist Diversified Life Care Partners Louisville, KY Job Details Part-time | Full-time $20 - $25 an hour 19 hours ago Benefits Mileage reimbursement Qualifications High school diploma or GED Driver's License Full Job Description Care Concierge & Thoughtful Engagement Specialist Part-Time (with Opportunity to Grow into Full-Time) | Louisville, KY & Surrounding Areas Position Summary Working under the guidance of our Care Management team, the Care Concierge & Thoughtful Engagement Specialist provides individualized support to older adults in their homes, assisted living communities, and other care settings. This position will begin on a part-time basis, with the opportunity to transition to full-time employment as client needs and program growth allow. This role is ideal for someone who enjoys building meaningful relationships, supporting seniors in daily life, and creating engaging activities that bring connection, joy, and purpose to each day. What You'll Do Client Support & Concierge Services Conduct wellness visits and monitor clients' overall well-being Provide transportation to medical appointments, errands, and social outings Accompany clients to appointments and provide support as needed Assist with medication refill coordination and communication with healthcare providers Provide light housekeeping and organization to maintain a safe, comfortable environment Assist with personal care needs such as grooming, dressing, and daily routines Report changes in client condition or concerns to the Care Management team Document services and client interactions accurately and timely Thoughtful Engagement & Enrichment Deliver personalized one-on-one engagement sessions based on each client's interests, abilities, and goals Create meaningful activities such as: Reminiscence and life story work Music and art activities Games and puzzles Sensory experiences Community outings Cognitive stimulation activities Utilize technology, including tablets, apps, and virtual reality experiences when appropriate Help reduce isolation and promote emotional well-being through meaningful connection Encourage participation in activities that support cognitive health, purpose, and independence Collaboration & Communication Work closely with Care Managers to support client goals and care plans Maintain professional communication with families, caregivers, and community partners Participate in team meetings, training opportunities, and case discussions Maintain client confidentiality and comply with all privacy regulations Qualifications High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare, social work, psychology, recreation therapy, education, art, or a related field preferred Experience working with older adults, individuals with dementia, or adults with disabilities preferred Background in healthcare, caregiving, senior services, recreation, social services, or a related field Experience as a CNA, Medical Assistant (MA), LPN, Activity Professional, Recreational Therapist, Social Services Professional, Dementia Care Specialist, or similar role is highly valued Knowledge of person-centered care and aging-related challenges Creative and engaging approach to helping seniors remain active, connected, and fulfilled Strong interpersonal, communication, and organizational skills Comfortable using Microsoft Office, smartphones, tablets, and electronic documentation systems Experience with cognitive engagement tools, activity planning, or technology-based enrichment activities is a plus Ability to work independently while collaborating effectively with a multidisciplinary team Compassionate, dependable, patient, and attentive to the needs of others Valid driver's license, reliable transportation, and current auto insurance required Ability to pass a background check and drug screening CPR/First Aid Certification or willingness to obtain Why Join DLCP?
