They are seeking an experienced, autonomous Supervising Physical Therapist (PT) to lead our mobility and restorative care initiatives. In this role, you will work beyond the constraints of traditional clinic walls, taking the time needed to build meaningful connections, conduct comprehensive in-home functional assessments, develop customized wellness programs, and mentor care team members. Conduct thorough in-home assessments covering whole-body mobility, gait, DME (durable medical equipment) needs, bed/car transfers, bathroom safety, and home accessibility (including entry and exit navigation). 30-
Day Reassessments:
Perform monthly reassessments to evaluate progress, adjust clinical plans, establish updated therapy goals, and identify emerging movement restrictions.
Routine Clinical Monitoring:
Check and record baseline vital signs (blood pressure, O2 saturation, heart rate, temperature, weight, and blood sugar as clinically indicated) during client touchpoints to maintain proactive health oversight. 2.
Care Planning & Program Design Goal Setting & Mobility Care Plans:
Establish individualized, client-centered mobility goals aligned with each individual's unique lifestyle, values, and functional aspirations.
Restorative & Wellness Exercise Programs:
Design safe, practical, step-by-step wellness exercise routines for Physical Therapist Assistants (PTAs) and non-clinical care team members to carry out between visits.
Dynamic Care Updates:
Continually adapt exercise and mobility regimens as client capabilities evolve. 3.
Clinical Supervision & Collaboration Team Guidance & Supervision:
Provide direction and oversight to Physical Therapist Assistants (PTAs) and caregiving staff, ensuring clinical plans are executed accurately and safely.
Internal Coordination:
Maintain prompt, clear documentation and team collaboration via internal communication platforms (e.g., Google Chat and clinical care systems).
Care Advocacy & Provider Liaison:
Communicate with external physicians, healthcare providers, and care partners when needed to coordinate care, relay clinical findings, and advocate for client needs. 4.
Actively identify environmental hazards or unmet physical needs during visits; assist with light functional environmental tasks as needed (clearing walkways, laundry, dishes) to ensure the living space actively supports safe movement.
Relational Caregiving:
Foster trust, dignity, and a positive "wow factor" in every interaction with clients and their family members.
Qualifications & Requirements Licensure:
Active, unencumbered Physical Therapist (PT) license in the State of Missouri.
Education:
Master's or Doctorate degree in Physical Therapy (DPT preferred).
Experience:
Minimum of 2-3 years of clinical experience, with strong background in geriatric rehabilitation, home health, neurology, or orthopedic care. Experience supervising PTAs and collaborating with multidisciplinary teams preferred.
Skills & Attributes:
Exceptional clinical reasoning and functional assessment capabilities. Strong relational intelligence, empathy, and patient-first communication. Self-directed, organized, and confident navigating clients' home environments independently. Reliable transportation and valid driver's license with active auto insurance.
