Job Description
Community Health and Wellness, LLC is looking for professional Aquatic Therapists to provide top notch services for individuals with disabilities throughout Texas. We have current openings in League City, TX. Contractors must have reliable transportation and a willingness to travel to consumer homes and facilities. Provide one-on-one Aquatic Therapy to children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses Create tailored goals to meet the needs of individual clients Document on each session Communicate with other team members and service providers Turn in timely documentation including weekly invoicing, progress notes, and related paperwork Essential Duties and Responsibilities Must be able to travel to current or prospective clients' homes Must be able and willing to transport client(s) during client visits Must be able to communicate with health care professionals, clients, families, and the general public through verbal and written communication Must have access to a private personal laptop or desktop computer for HIPAA compliance Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader Implement patient care plans through home and community goal-related activities Able to work independently, manage one's own time and the time of client(s) All therapists must have the following: Resume CTRS or TRS/TXC or LMT certified
CPR/AED/FIRST AID
Certification (American Red Cross or American Heart Association only) Basic Water Rescue and/or Lifeguard Training Social Security Card Driver's License Auto Insurance (with 100/300/100 coverage minimum) Professional Liability Insurance (with $1 million/$3 million coverage limit)
