One person being hired for every weekend 3-11pm, and another role being filled for

30pm. These two dynamic roles combine providing attentive patient care with maintaining a clean, safe, and welcoming home environment. You will play a vital part in enhancing the quality of life by delivering comprehensive assistance tailored to their unique needs, fostering independence, dignity, and comfort in their daily routines. Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, skincare and personal hygiene to promote independence and well-being. Monitor patients' health by observing vital signs, recognizing changes in condition, and reporting findings promptly. Following scheduled care routines and documenting thoroughly so all members of team can follow the plan timely. Medication reminders, following care plans and ensure proper documentation in compliance with HIPAA regulations. Prepare nutritious meals tailored to dietary requirements and assist with feeding as needed to support optimal health. Perform housekeeping duties including cleaning, laundry, organizing living spaces, and ensuring a safe environment for clients. Support clients with mobility using appropriate transfer methods and equipment such as Hoyer lifts or patient transfer techniques. Also will be required to drive handicapped van with clean driving record. Provide transportation assistance for medical appointments, errands, or social activities to promote community engagement. Maintain accurate records of patient observations, care provided, and any incidents to ensure continuity of care and compliance with necessary medical updates to care team. Engage in patient observation for signs of illness as they are both on the autism spectrum, or other medical changes to support specialized care needs. Follow all safety protocols including first aid, Heimlich maneuver, manual handling procedures, and resident rights to uphold a secure environment. Skills Proven experience working with individuals with complex medical and emotional needs. Strong knowledge of Autism, paraplegia, foley and ileostomy care. Ability to perform patient monitoring accurately while demonstrating excellent communication skills with clients and team members. Will need to have Federal background check done prior to interview. Skilled in assisting with ADLs such as meal preparation, personal hygiene, bathing and skincare, medication reminder, and housekeeping tasks. Competence in manual handling techniques including heavy lifting and patient transfers using appropriate equipment like Hoyer lifts. Experience supporting individuals with physical disabilities and emotional disabilities with patience and understanding. Ability to respond effectively during emergencies through first aid knowledge and facilitation of transition to hospital if needed. Excellent organizational skills for managing supplies, appointments and documenting patient updates and medical changes meticulously.
