Job Overview The Sycamore of River Falls is a growing, we are a full-service senior living community with 84 apartments including 68 blended independent and assisted living and 16 memory care apartments in a secured neighborhood on first floor. The property has four villa patio twin home units with two bedroom and three bedroom options for active, independent seniors. The apartment building consists of a variety of studios, one-bedroom, one-bedroom plus den, and two-bedroom units. The underground parking garage has 50 parking stalls, maintenance rooms, and residents' storage area. Managed by Ebenezer. This newer care facility is Located in River Falls, WI This position is responsible for providing personal care and designated health-related services to enhance and maintain each resident's physical, emotional, social, and spiritual well-being. In addition to direct resident care, this position serves as a shift lead, supporting the coordination of daily operations, promoting accountability, and ensuring a consistent, resident-centered experience across clinical care, dining, housekeeping, and life enrichment. They assist with planning, implementing, and evaluating Life Enrichment and Dimensions programming to optimize residents' physical, psychosocial, spiritual, and cognitive well-being. They provide exceptional dining room service, help maintain a clean, safe, and welcoming environment for residents, staff, and visitors, and serve as a resource and coach to Resident Experience Specialists by fostering teamwork, modeling best practices, and collaborating with department leaders to ensure high-quality service and operational excellence. Responsibilities Assists residents with personal cares including but not limited to: Bathing/showering Foot and fingernail care Shaving Dressing Assistance with use of hearing aids and/or glasses Hair care Feeding Toileting Escorts 1-2 person transfers and use of mechanical lift Ensure all services are completed on service schedules prior to ee leaving. Assist with move-ins Verifying supplies are stocked Help schedule and attend Care Conference Conduct beginning-of-shift huddle with RES team. Recommend service schedule adjustments based on resident acuity. Coordinate coverage during breaks and call-ins. Be a part of the staffing on-call rotations. Ensure smooth handoff between shifts. Train new RESs. Help coordinate and conduct on-site orientation.
Provides health-related services as directed:
Assists with treatments per individualized care plan under the direction of the Registered Nurse. (Includes blood glucose monitoring) Administers medication or assists with this under the direction of the Registered Nurse (includes insulin injections). Takes and records temperature, blood pressure, pulse and respiration under the direction of the Registered Nurse. Assists with life enrichment programs and activities: Leads social events, activity programs, and wellness programming. Documents as required. Offers suggestions and ideas to the Executive Director
  • Resident Experience and Operations to improve the programming. Assists with the planning and execution 1:1 and small group informal activities as directed by leadership or observed as needed by residents. Print & Communicate Activities Daily

Identify residents needing one-to-one engagement Coordinate incoming vendors Help mark completion of activities through activities portal Track residents who consistently decline participation Monitor music, lighting, aromas, and cleanliness Ensure beverages/snacks are available Encourage and embrace use of Dimensions program: Attend Dimensions Trainings Utilize best practices of the Dimensions Program to ensure effective approach and interaction with persons living with dementia. Collaborates with team to maximize recommended contributions to resident care plan from current Cognitive Performance Tests (CPT), My Way, Advance Care Planning and Global Deterioration Scale (GDS). Ensure Dimensions environment is conducive to enhancing resident experience and encouraging engagement. Offers suggestions and innovative ideas to improve the Dimensions Community to the Executive Director
  • Resident Experience and Operations.
Serves meals and ensures that the dining area is clean and inviting: Follows meal serving schedule and procedures. Offers resident meal choices and meets resident's specific dietary needs and preferences. Uses proper safety and sanitation techniques. Follows dining room cleaning schedule and procedures. Follow proper procedures for dining room set-up. Welcomes residents and initiates conversation. Follows proper work flow to stay on task. Learns and becomes proficient with EMenu System and restaurant style service. Ensures all dishes (including flatware, drinkware, etc.) are washed, sanitized, and put away before end of shift. Accommodates resident requests such as requests for food substitution, change in seating, etc. Assists with meal prep. Delivers meal trays. Complete pre-meal dining walkthrough Audit to ensure all meals are entered through culinary software Confirm tables are set consistently Ensure beverage station is stocked Serve as greeter and initial meal support in the dining room Coach restaurant-style service Ensure aromatherapy hand and face cleaning after every meal

Ensure post-meal cleanup is completed Communicate resident concerns to Culinary Director Ensure completion of Daily Checklist Ensure beverages/snacks are available Cleans resident and common areas of the community as assigned: Performs thorough cleaning of all assigned areas, including vacuums carpets, sweeps and mops hard floor surfaces, dusts and/or damp wipes surfaces, sanitizes bathrooms, cleans all glass windows/mirrors. Safely mixes chemicals according to instructions provided by the manufacturer Demonstrates precautionary work practices for the safety of others in the work areas, such as electric cords, wet floor signs, etc. Prioritizes tasks and completes them within allotted time. Assists in cleaning spills and accidents as needed. Follows cleaning schedule and instructions regarding specifications of resident's individualized housekeeping needs. Submit work orders in TELS to ensure timely response to resident requests. Maintains housekeeping cart and storage area regularly. Safeguards others' well-being by keeping chemicals and other hazardous materials locked when not in active use. Communicates supplies that need to be re-stocked. Assists with light maintenance tasks as needed. Complete daily environmental rounds Identify immediate cleaning needs Ensure housekeeping schedules are completed Monitor resident apartment readiness for move-ins Encourage submission of work orders through TELs Verify housekeeping carts remain organized and stocked
