Job Description
Summary/Objective Delivers day-to-day activity support under the direction of the Activity Supervisor and guidance from Activity Lead. Essential Duties/Responsibilities Assist with set-up, facilitation, cueing and redirection for groups and 1:1s. Tailor approaches cognitive and physical abilities. Provide companionship and 1:1 engagement for participants who are new, low-engagement, or at risk of isolation. Observe and document attendance/participation, promptly report changes in condition or behavior to the Activity Supervisor or Lead. Assist with safe ambulation/transfers within scope and training; maintain environmental safety and infection control. Prepare material, manage light cleanup, and support transportation transitions and meal-time activities as assigned. Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies. Communicate with supervisor on daily basis, and as needed, to report all areas of concern/issues/incidents. Demonstrate professionalism in relationships with customers/co-workers/supervisors to build rapport while refraining from inappropriately familiar personal relationships Complete and attest to accuracy of payroll records, and submit for review and processing according to agency schedule Serves as a resource to staff and coordinates volunteer activities at the center. Required Education and Experience High school diploma, or general education degree (GED; prior experience in senior services, adult day, or long term care preferred. Effective communicator; dependable; able to follow care plan directions and safety protocols. Preferred Education and Experience Related experience working with older adults. Experience in Healthcare, or Medicaid/Medicare setting.
