Join our team where your skills, compassion, and commitment are truly valued. At Apple Valley Village Health Care Center , you'll be part of a supportive, mission-driven team that takes pride in making a difference every day for residents and families alike. In this role, you will meet with new transitional care patients to conduct assessment interviews, provide activity resources, and help them have a good experience during their stay. Our ideal candidate is self-motivated with a genuine concern for our residents' well-being. Must be organized, detail oriented, with meticulous documentation and communication skills, and have the ability to complete documentation accurately and within established timelines. This position requires a high volume of documentation for each interview completed.
