The following responsibilities are not intended to serve as a comprehensive list of all duties performed by all employees in this position, only a representative summary of the primary duties and responsibilities. Incumbent(s) may not be required to perform all duties listed and may be required to perform additional, position-specific duties. Working with team members and residents as well as drawing from assessed resident preferences and choices, design and implement a dynamic, engaging and meaningful activity program that caters to social, cultural, educational, wellness, recreational, occupational, creative and spiritual needs. Provide one-to-one programming for residents who cannot participate in a group setting. Complete assessments, monthly notes and other documentation as directed. Ensure that documentation adheres to state and federal guidelines. Plan and run activities that encourage socialization and relationship development in accordance to the Social Services Director. Engage minds and bodies with a variety of social, recreational, occupational, physical, creative and spiritual programming. Chart the activities you lead as directed. Work with residents, staff members and the general public to grow a sense of community within the home and maintain their connectedness to the world-at-large. Provide monthly calendars to all residents and monitor facility calendar as directed.
Desired Experience and Education:
Basic computer skills, such as Microsoft Office required. Completion of Oklahoma certified activities training program preferred. One or more (1+) year of activities experience in a healthcare setting is strongly preferred. One or more (1+) year of work in a skilled nursing facility, LTC, hospice or LTAC preferred. CNA Certification helpful but not required.
