Pubic Health Project Coordinator-(Behavioral Health Integration Specialist) The Behavioral Health Integration Specialist supports individuals referred through community outreach by providing case management, carecoordination, linkage to services, and ongoing follow-up. The position serves as a bridge between outreach activities and behavioral health services, ensuring clients receive appropriate referrals, support, and follow up to remain engaged in care. Receive and review referrals from Community Outreach Specialists and other community partners. Engage clients, assess needs, and develop individualized plans for connecting to services. Connect clients to behavioral health, substance use, healthcare, public health, housing, and other supportive services. Build trust and maintain ongoing contact to support engagement, continuity,and retention in care. Re-engage clients who have become disconnected from services. Identify barriers to care and help clients navigate solutions. Coordinate with outreach workers, peer support staff, clinicians, case managers, and community partners. Document services and client interactions accurately and confidentially. Identify unmet needs and service gaps and communicate them to program leadership. Support a client-centered, trauma-informed, and culturally responsiveapproach to care. Perform other duties and responsibilities as assigned by the manager. Bachelor's or Master's degree Public Health, Business Administration, Health Science Administration or other related field, with two years experience in community engagement, community organizing, and project coordination and implementation. Equivalent combinations of education and experience may be substituted to meet the education and experience requirement of this position.
