Senior Community Services Assistant (Outreach Specialist) The Community Outreach Specialist connects individuals and communities to behavioral health, public health, substance use, prevention, harm reduction, and supportiveservices. The position builds community partnerships, conducts outreach and education, provides referrals and linkage to care, and supports client engagement and retention in services. Conduct community outreach and engagement activities in neighborhoods and community settings. Build and maintain relationships with public health agencies, community organizations, healthcare providers, schools, faith-based organizations, public safety agencies, and other partners Provide education on behavioral health, public health, substance use, overdose prevention, harm reduction, and available community resources. Provide referrals and linkage to appropriate behavioral health, public health, substance use treatment, recovery, crisis, housing, and other supportive services. Provide warm handoffs to Behavioral Health Case Manager and assist clients with navigating the referral process. Support client engagement and retention through ongoing outreach, follow-up, appointment reminders, resource navigation, and supportive services. Re-engage clients who have disengaged from services and assist them in reconnecting with appropriate providers. Track referrals, successful linkages, client engagement, and retention outcomes. Identify community health needs, barriers to care, and service gaps and communicate findings to program leadership. Support stigma-reduction and culturally responsive outreach efforts. Participate in multidisciplinary meetings and community planning activities. Assist with targeted outreach to individuals and communities identified as being at increased risk for behavioral health concerns, substance use, overdose, or violence. Maintain confidentiality and perform all duties within organizational policies and scope of practice. Perform other duties and responsibilities as assigned by the manager. Qualifications (required): Bachelor's degree in the social sciences, education, psychology, guidance and counseling or related field. One (1) year of recent experience as a supervisor, preferably in a social, economic or community development program. Three (3) years of experience in case work, group work, individual and group counseling and guidance, social research or community or public relations. Preferred Knowledge of community engagement principles and practices. Excellent communication and interpersonal skills with the ability to engage diverse populations. Familiar with community resources and organizations. Strong organizational skills and the ability to manage multiple projects simultaneously. Ability to work independently and as part of a team, and to collaborate with various stakeholders. Experience with organizing and coordinating community events, workshops, or informational sessions. Weekends/evenings may be required. Cultural sensitively and the ability to work effectively with individuals from diverse backgrounds. Equivalent combinations of education and experience may be substituted to meet the education and experience requirement of this position.
