Are you ready to make a real impact in the lives of individuals with disabilities? Do you have a passion for helping others reach their fullest potential? If so, New Avenues to Independence is looking for someone just like you to join our team as an Inclusion Services Specialist!
Position Overview:
As an Inclusion Services Specialist at New Avenues to Independence, you will have the incredible opportunity to support and empower individuals with disabilities, helping them lead fulfilling lives and become active members of their community. This is more than just a job; it's a chance to make a difference every single day.
Location:
Cleveland, OH Pay rate: $17 per hour.
Shift:
Hours vary What you'll do: Accompanies individuals to job sites; observes performance; oversees work training; provides job retention training, instruction and assistance accordingly. Provides support to Individual and team members in obtaining participant's information to assess employment history, education, skills and abilities and potential barriers to employment. Functions as an advocate and customer service liaison between site personnel and individuals to maintain open communication and achieve community inclusion goals. Ensures adherence to HIPAA standards at all times. Provide person-centered care and support to individuals with disabilities in a variety of settings, including residential homes and day programs. Assist individuals with daily living activities, such as meal preparation, personal hygiene, and medication management. Foster a nurturing and inclusive environment that promotes growth, independence, and self-determination. Plan and engage in meaningful activities, outings, and events to enhance the quality of life for the people you support. Promote social and emotional well-being by building strong, supportive relationships. Collaborate with a dedicated team of professionals to create and implement individualized care plans. Collaborating with people to complete tasks and build independence in their lives. Providing support with daily routines such as personal care, cooking, cleaning, shopping and attending medical appointments.
Requirements:
What We're Looking For:
