This role helps ensure appropriate staffing coverage, accurate service schedules, and safe, consistent care for residents.

The Glenn Hopkins - Hopkins, MN Job Responsibilities Staffing & Scheduling Coordinate clinical staff schedules with the RN/DON, including open shifts, call-offs, time-off requests, and coverage needs. Proactively identify staffing gaps and communicate schedule changes to staff and leadership. Maintain accurate staff schedules, availability, and scheduling records. Clinical & Health Office Support Verify resident service schedules and make updates under RN/DON supervision. Maintain service agreements, resident charts, progress notes, and clinical documentation in accordance with regulations. Enter provider orders and new resident information; assemble new charts, thin active charts, and process discharged charts. Order labs, complete lab slips, collect non-catheter urine samples, and coordinate specimen delivery. Order imaging exams, including X-rays and ultrasounds, as directed. Retrieve and process communications from systems such as CentraCare Link and Bluestone Portal. Assist with Express Scripts ordering, provider rounding forms, and medical supply/equipment orders. Coordinate with Rehab regarding therapy start/end dates, recommended exercises, and staff follow-through. Monitor completion of requested resident blood pressures, weights, and pulses. Report changes in residents' physical or mental health status and resident/family concerns to appropriate clinical staff. Resident & Team Support Make daily rounds and communicate pertinent resident information through appropriate systems, meetings, or communication tools. Coach Resident Assistants on care procedures, resident engagement, activities, and successful one-to-one interactions. Encourage resident participation in activities based on individual needs and preferences. Provide education and support to staff regarding successful interactions and interventions for residents with dementia. Provide meal supervision as needed and promote a positive, team-oriented work environment. Attend staff meetings and work with the DON to maintain emergency procedures. Report accidents/incidents and respond to emergencies according to community procedures. Support regulatory compliance and participate in state and federal surveys as needed. Maintain flexibility to respond to clinical concerns or emergencies outside regular hours when needed. Follow company policies and perform other duties as assigned. Qualifications One year of experience in home health, assisted living, long-term care, or a related healthcare setting preferred. Staff scheduling or staffing coordination experience preferred. Strong organizational skills with the ability to take initiative and follow through with minimal supervision. Strong judgment, problem-solving, and decision-making skills. Excellent verbal and written communication skills. Strong computer skills, including electronic resident records and staff scheduling software.
