Job Description:
Fort Drum Regional Health Planning Organization (FDRHPO) is seeking a dynamic, self-motivated individual to support its Health Workforce program. The Workforce Case Manager will facilitate regional workforce development throughout Jefferson, Lewis, and St. Lawrence counties to include mentorship, education, and job placement. This position will provide intensive one-on-one support to assist individuals in establishing healthcare career pathways, develop technical skills needed for career advancement, and connect individuals to job seekers.
Responsibilities:
Develop and foster relationships with collaborating partners, including educational institutions, healthcare providers, community organization, and clients. Provide personalized support to help clients define educational and career goals and help develop individualized career enhancement plans. Assist students in the job placement processes upon completion of their programs. Maintain detailed and accurate records of student progress and participation in program activities. Lead regional implementation of the Institute for Career Advancement in Medicine (ICAM) platform, a career pathway and training platform that supports recruitment, retention, and advancement of the healthcare workforce. Proactively assess individual barriers (i.e. transportation, childcare) and provide warm handoffs to community agencies to address these barriers to keep clients engaged in training and employment. This position may require local travel throughout Jefferson, Lewis, and St. Lawrence Counties to include attendance at meetings which may go beyond usual working hours. Assist with special projects and other duties, as needed.
Qualifications:
Experienced professional with initiative, who is goal-oriented and outcomes-driven. Associate's degree in business, program management, healthcare, human resources, or a related field with relevant experience; a bachelor's degree is preferred. Familiarity with the challenges faced by post-secondary students, including those from diverse and underrepresented backgrounds. Proven ability to be self-driven and independently manage a large caseload with exceptional organizational detail and adherence to deadlines. Exhibits excellent communication, teamwork and interpersonal skills with a professional demeanor. Demonstrated capacity for empathetic communication when handling client information and personal circumstances.
