AFC Group Home Supervisor

Creative Lifestyles is looking for a Supervisor in Warren. This employee plays a vital role in advancing our mission to make a difference in the lives of our residents. The facility is a licensed Adult Foster Care Home (6 residents). This is a Full time salaried position with changes to working hours based on the needs of the facility. Interested candidates should have a minimum of 2 years of direct care/direct support experience and at least 1 year of Medical Coordinator or Assistant Supervisor experience. This role is ideal for someone who enjoys being a leader with a high level of responsibility, is comfortable working alone or with others, and realizes that frequent communication is the key to success!

Duties include:

Daily communication with and following directives from company Administrator. Creating monthly work schedules for staff. Medications- reviewing records and ensuring staff follow proper Medication Administration. Correspondence with Doctors and Nurses. Frequent communication with Direct Care Staff and Assistant Supervisor. Documentation requirements including completing, obtaining signatures, and proper organization/filing of documents. Attending plan of service meetings with residents and their care team. Reviewing and processing payroll on a bi-weekly basis. Communication with Case Managers, Guardians, and family members of residents. On-call 24/7 for all medical needs of the residents and staffing issues Reviewing Plans of Service and ensuring that current documents are on file for each resident. Training of new hires and existing staff. Direct Care work including medication passing, meal preparation, and housekeeping. Scheduling regular activities for the residents. Preparing weekly menus, creating shopping lists, and completing grocery shopping. Transporting residents to and from work sites using a company provided van. Reviewing and completing a home maintenance checklist on a monthly basis Afternoon, night and weekend shifts will be required when covering staffing shortages, absences, or scheduled vacations Other responsibilities to be discussed. Online Training will be provided if not current with Macomb County's training requirements. Must pass all training requirements. Responsible for ensuring all documentation and on-site requirements are met for frequent audits. Must have a good driving record and valid Michigan driver's license
