Job Description

ClareMedica Health Partners LLC seeks a Clinical Trainer to support our physician-led primary care practices serving Medicare and senior patients across South Florida. In this role, you will design, deliver, and continuously improve training on clinical workflows, EHR use, quality measures, and value-based care. Partnering closely with physicians, nurses, and care teams, you will lead onboarding, conduct in-person and virtual training, and coach staff to ensure consistent, patient-centered practices. You will help drive preventative, coordinated care, supporting better health outcomes, access, and satisfaction for our senior population. Responsibilities Design and update training programs on clinical workflows, EHR use, and quality standards for Medicare and senior care. Deliver in-person and virtual training sessions for clinicians and staff across neighborhood medical centers. Assess training needs using performance data, feedback, and quality metrics to prioritize learning initiatives. Onboard new clinical staff and support role-specific competencies and standard operating procedures. Coach and mentor team members to reinforce best practices in preventative, value-based primary care. Ensure training content aligns with regulatory, compliance, and organizational quality standards. Collaborate with physicians, nurses, and operations leaders to standardize workflows and patient care processes. Track and report training participation, competency completion, and impact on care quality and patient outcomes.
