Our Psychosocial Rehabilitation helps individuals with complex mental health needs develop social, vocational, and life skills to improve independence and quality of life.
Job Scope:
The PSR Group Facilitator is responsible for planning and leading daily group programming for clients with serious and persistent mental illness, including Wellness Center, Community Support, and Co-Occurring groups. The role includes completing assessments and treatment plans, providing supportive counseling and crisis intervention, and ensuring services meet Medicaid Rule 140/132 standards. The facilitator documents services within 48 hours, maintains billing expectations, assists in training interns, and collaborates with the PSR Supervisor to support departmental goals. This position also engages in outreach, attends required meetings and trainings, and upholds the agency's mission and values. Primary Responsibilities Facilitate groups throughout the week for intensive programming in the PSR department Must be able to complete assessments, treatment plans, and progress notes according to agency policy Participate in running Wellness Program and adjust programming according to client needs Provide counseling to those in PSR program Participate in other PSR events and responsibilities Performance Requirements Understanding Recovery and having the ability to clearly define Recovery for people possessing a wide range of skills and abilities. Ability to lead a module-based skill development group. Organizing and facilitating a variety of on-site and off-site activities (CSG Groups) which enable people to develop relationship skills, to learn constructive hobbies and to function with increasing levels of independence. Ability to establish trust and rapport in relationships with PSR/Wellness clients. Ability to impart information in a clear and understandable fashion. Sufficient knowledge of mental illness and human behavior to be a teacher, an intervention agent when necessary, and a provider of clinical information to other providers and surveyors. Education and Experience Bachelor's degree in human service field One year clinical experience PSR experience preferred Valid Illinois driver's license, insurance, and automobile Schedule Tuesday, Thursday, and Friday, 8:30am - 4:00pm (21 hours a week).
