Job Description

Description:
Supports the Mission, Values and Vision of Lydia Care Center and the facility. Is knowledgeable of consumer rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all consumers in a safe, secure environment. Supports, cooperates with, and implements specific procedures and programs for: o Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs. o Confidentiality of all data, including consumer, employee and operations data. o Quality Assurance and compliance with all regulatory requirements. Supports and participates in common teamwork: o Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment. o Uses tactful, appropriate communications in sensitive and emotional situations. o Follows up as appropriate with supervisor, co-workers or consumers regarding reported complaints, problems and concerns. o Promotes positive public relations with consumers, family members and guests. o Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.
Requirements:
o Schedule weekly clinical supervision of MHP and RSA team members, followed by submission of weekly progress reports of training, coaching, assessments, as well as overview of clinical supervision; o Pursuant to facility policy, shall debrief the staff involved in restraint use regarding the events leading up to the restraint, physical hold, or application as well as the continued assessment of consumers condition during the duration of the restraint use; including upon return to the facility if transported out; o Shall develop or supervise the development of discharge plans; o Assist in maintaining appearance of unit and consumers; o Work in conjunction with the CRSS team to plan, coordinate and care for each consumer; o Provide assessment, individual and group therapy, and other mental health services as needed; o Must be able to provide on-call job rotation to handle any potential emergency that may arise in the facility; o Must be able to provide clinical services, treatment planning and therapy for 60 minutes per consumer per week (shared duty); and o Provide group recreational and rehabilitative services for 60 minutes per consumer per week (shared duty); o Must be able to continue ongoing training and development as outlined by the facility and the DHSDMH; o Provide the identified consumers and their families with additional time/education to explain the services offered at Lydia Care Center, answer questions and provide reassurance when needed.
