Or both shifts can be one person if they would like. She just needs help putting a compression sock on and off for evening and light help with household stuff. If certain days work but not all please still apply! As well as times can be flexible. In this vital role, you will provide essential support to clients within their homes or community settings, promoting independence, safety, and well-being. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs and observing for any changes in condition Administer medications accurately following care plans and HIPAA confidentiality standards Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support clients with cognitive impairments including Alzheimer's disease, dementia, or memory care needs Use specialized equipment like Hoyer lifts for safe patient transfers and heavy lifting when necessary Maintain detailed documentation of care provided, patient observations, and updates to care plans Engage in patient monitoring to ensure safety and comfort during daily routines and activities Skills Proven experience in home & community care, assisted living, or long-term care environments Knowledge of HIPAA regulations to ensure patient privacy and confidentiality Certification in CPR and First Aid to respond effectively during emergencies Experience working with individuals with developmental disabilities, autism, or dementia-related conditions Strong ability to observe patient conditions closely and report changes accurately Skilled in medication administration, vital signs measurement, and basic nursing procedures Ability to perform light housekeeping tasks such as cleaning and maintaining a safe environment for clients Compassionate communication skills to support patients' emotional well-being and foster trust Join us in providing exceptional home care that enriches lives!
