Individual Community Living Supports (ICLS)

Position Summary:

The ICLS Staff will provide personalized support to a client in both their home and the community. This role focuses on fostering independence, ensuring safety, and assisting with daily living needs while engaging the client in meaningful community activities.

Qualifications:

 Must be at least 18 years of age.  Valid driver's license required.  Must have a reliable vehicle.  Must be bilingual in Hmong and English.  Female applicants only (client is female and speaks only Hmong).

Key Responsibilities:

 Manage the client's calendar, appointments, and reminders.  Assist with Activities of Daily Living (ADLs) tasks and routines.  Provide transportation to appointments, errands, and social activities.  Assist with shopping, errands, and picking up medications (with reminders).  Translate and explain mail and documents.  Support proper use of the UCare health card.  Assist with household organization, laundry, and kitchen cleaning.  Support participation in community and social activities.  No weekends required (occasional weekends may be needed)  Total hours per two weeks: 20 hours

Pay Rate:

 $16.00 - $20.00 per hour, depending on previous Home Care experiences
