Join Our Team! Are you a passionate and dedicated Physical Therapist looking to make a real difference in patients' lives? Delta Health is seeking a Lead Physical Therapist to join our dynamic Rehabilitation Services team. You'll work with a diverse patient population—from pediatrics to geriatrics—helping individuals regain mobility, independence, and improve overall quality of life. About the
Administer physical therapy treatments, including exercises, therapeutic modalities, and use of assistive devices.
Guide and support patients through therapeutic activities to restore mobility, function, and independence.
Monitor patient progress and adjust treatment plans as needed.
Contribute to discharge planning and create home exercise programs.
Maintain accurate documentation of evaluations, treatments, and patient progress.
Ensure a safe, clean, and compliant work environment.
Supervise Rehab Aides and collaborate with other team members to ensure continuity of care.
Stay current with professional knowledge through workshops, publications, and continued education.
BLS Certification required.
Previous physical therapy experience preferred.
Strong knowledge of therapeutic practices, rehabilitation procedures, and patient safety protocols.
Excellent interpersonal, communication, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
Capable of maintaining manual dexterity for operating therapy equipment and computers.
Ability to sit for extended periods and perform medium-level physical effort tasks. Why Join Delta Health? Role:
Under the supervision of the Rehabilitation Services Director, the Lead Physical Therapist is responsible for administering comprehensive physical therapy services to patients across the lifespan. You'll collaborate with interdisciplinary teams, develop and implement treatment plans, and support patients in achieving their functional goals.Key Responsibilities:
Assess and evaluate patients to develop individualized treatment plans in coordination with physicians.Administer physical therapy treatments, including exercises, therapeutic modalities, and use of assistive devices.
Guide and support patients through therapeutic activities to restore mobility, function, and independence.

Qualifications:
Current Physical Therapy License issued by the State of Colorado Department of Regulatory Agencies.BLS Certification required.
Previous physical therapy experience preferred.
Strong knowledge of therapeutic practices, rehabilitation procedures, and patient safety protocols.
Excellent interpersonal, communication, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
Physical Requirements:
Ability to walk, bend, stoop, squat, and lift up to 50 pounds occasionally and 20 pounds frequently.Capable of maintaining manual dexterity for operating therapy equipment and computers.
