Provides a clinically based concurrent and retrospective review of inpatient medical records to evaluate the documentation of acute care services. Generates and issues compliant queries to providers for conflicting imprecise incomplete or inconsistent clinical documentation. Initiates contact with physicians to obtain complete and timely query responses.
