Primary Responsibilities:

Family Engagement:

Actively engage and collaborate with families to support their needs and promote participation in services.

SOC Collaborative Participation:

Take part in System of Care (SOC) Collaborative meetings and initiatives to enhance community-based service delivery.

Screening and Assessments:

Conduct QUAD Screeners for all Mecklenburg County clients, with enhanced screening for those flagged during SOC assessments.

Follow-Up Care Coordination:

Provide first-time follow-up for all enhanced referrals from intake and engagement processes, covering all Mecklenburg County clients as well as out-of-county referrals.

Referral Assistance:

Facilitate and ensure successful connections for clients referred to external organizations.

NOMS Completion:

Conduct National Outcome Measures (NOMS) assessments with families at designated intervals to track progress and outcomes.

Ongoing Engagement Support:

Collaborate with internal supervisors and teams to address any issues that arise post-service initiation and ensure continued client engagement.

Service Linkages:

Assist clients in accessing services and addressing needs identified through Social Determinants of Health (SDOH) screenings.

Education and Empowerment:

Educate and empower parents, caregivers, and/or youth on effectively navigating child-serving systems, helping them understand their options and access both formal and informal community resources.

Resource Facilitation:

Assist families in accessing and utilizing available resources, including medical, educational, social, therapeutic, and other services to meet their unique needs.

Service Coordination:

Identify, facilitate, and monitor the delivery of necessary services, ensuring that families and youth receive appropriate and timely support.

Collaboration:

Work in partnership with service providers, state agencies, and other relevant entities to advocate for and meet the needs of youth and families.

Family Support Partner:

Must have lived experience as a primary caregiver for a child who has/had mental health, substance use disorders, or intellectual/developmental disability. Experience in navigating any of the child and family-serving systems and teaching family members who are involved with the child and family serving systems Holds National Certification in Family Peer Support or willing to obtain certification and is on track to complete Family Peer Support certification within one year of hire date. Family Partner 101 is part of National Certification Trainings for North Carolina.
