Job Description

The Youth Matter Family Support Partner (FSP) provides person-centered mentoring and support to adolescents and their families in our adolescent intensive outpatient program (IOP). This role helps families with system navigation, emotional support, and connection to community resources. The FSP serves as a primary point of contact for adolescents receiving IOP services and their families. This role will help families navigate challenging situations, identify and engage natural supports and build skills to increase self-efficacy in recognizing substance use disorder as a family disease. The FSP works as an integral part of the interdisciplinary treatment team and coordinates with clinical staff in the program. The hours will vary, but the schedule will typically be three times a week, around 5:00 PM-9:00 PM
ESSENTIAL DUTIES AND RESPONSIBILITIES
Develop effective, trusting relationships with families and adolescents using a person-centered approach and best practices in the field. Offer mentoring and support to families by providing them with practical guidance on navigating behavioral health, education, juvenile justice and social service systems. Partner with families and adolescents to advocate for needs, promote self-advocacy, and ensure services are family driven. Communicate with families to determine level of understanding of their adolescent's diagnosis and situation to offer relevant education and information. Monitor client progress and maintain contact with internal and outside entities to ensure continuity of care and adherence to the treatment plan. Maintain a high level of engagement with all clients on your caseload to track progress and needs and promote an environment of recovery. Participate in weekly interdisciplinary treatment team meetings and treatment planning discussions to ensure integrated and collaborative care. Maintain timely and accurate documentation in accordance with DBHDS, Medicaid, and agency standards. Conduct outreach and engagement activities to retain clients in outpatient services and reduce treatment dropout. Facilitate group activities and maintain attendance sheets accordingly. Engage in effective discharge planning meetings with clients and ensure timely completion of discharge summaries in the EHR. Utilize trauma-informed, recovery-oriented, and culturally competent approaches in all aspects of service delivery. Adhere to all confidentiality, ethical, and professional standards, including HIPAA and 42 CFR Part 2. Serve as a positive ambassador of the agency by modeling professionalism, fostering a collaborative and respectful work environment, and contributing to the organization's positive reputation in the community. Other duties as assigned.
MINIMUM QUALIFICATIONS
(KNOWLEDGE, SKILLS, AND
ABILITIES
) Virginia CPRS with lived and living experience with complex behavioral health issues in general and as a caregiver of adolescents with behavioral health issues Experience working with children and families Experience working in a behavioral healthcare service setting Valid driver's license Self-motivated Availability to work evenings. Good communication and organizational skills Ability to maintain program requirements and boundaries with compassion and respect Problem solving abilities Ability to complete and submit documentation and reports
Pay:
