Full Job Description Position Summary:
The Family Care Coordinator Associate is responsible for case management including home visits and classroom support, as well as program support including recruitment and transportation coordination. This role carries a smaller caseload and participates in recruitment and enrollment activities, ongoing case management, classroom support, and connecting children and families with resources and services including.
Responsibilities:
Participate in recruitment and enrollment activities. Arrange and promote recruitment activities including presentations to community partners Complete enrollment paperwork including designated screening forms Observe the parent-child interaction during intake through enrollment phase. Provide ongoing case management. Provide home visits with caregivers at least monthly to address family needs, parenting skills and behavior planning consistent with the classroom and Family Care Plan. Coach caregivers on responsive parenting, child development, and social-emotional learning. Collaborate with caregivers, classroom staff and Mental Health Consultant. Arrange multidisciplinary meetings every 90 days and Maintain consistent communication with families to monitor progress and provide ongoing support. Monitor family's need for ECLIPSE transportation, arrange for bus service if needed and document in child record. Support behavior strategies provided by the MHC within classroom and home settings. Provide classroom observations and support; follow up with classroom staff regarding interventions, children's progress or concerns, and family needs. Make referrals to community services whenever necessary. Maintain a caseload of 10-16 families. Coordinate ECLIPSE bus services for the program. Communicate program needs with People for People for transportation services Maintain ongoing communication with childcare staff, Family Support Staff and Program Managers for changes and updates Maintain bus schedule/route information and distribute, as required Complete all documentation according to agency policy and required by the contract. Document home visits, community meetings, MDT meetings and other contacts on calendar daily Complete documentation within 48 hours of scheduled visits Keep ELMS/Documentation tracker updated, as required Attend and plan for transition meetings in conjunction with Mental Health Consultant upon family's approach and exit from ECLIPSE Document all phone calls, case notes, contacts with allied service providers and other treatment information in the electronic record according to the ECLIPSE policies and procedures. Maintain communication with assigned social workers and referral sources regarding progress and concerns. Follow up with classroom staff to make sure objective assessments are completed and scored on time. Meet productivity standards of 50% of time in direct face-to-face services including home visits, multidisciplinary team meetings, case management, classroom observations, etc. Maintain confidentiality according to HIPAA and FERPA in all aspects of the program Actively participate in team operations and supervision. Communicate directly with supervisor for any questions or concerns regarding program design/implementation, job requirements, behavior interventions, training or general program needs. Staff cases that require team coordination or therapeutic input with Mental Health Consultant and/or Program Manager, as needed Organize and participate in family events, support groups, and parent meetings Perform other duties as assigned
Job Requirements:

Physical Requirements:
This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring medium physical exertion, talking and hearing and adequate vision on a regular basis to perform the job requirements. These physical demands are required up to 60% of the time. In addition, approximately 40% of the time may include lifting and carrying children, moving quickly around the room to maintain appropriate supervision and safety and sitting on the floor or in small chairs with the group.
Non-physical Requirements:
Education Associate's degree in early childhood education, human development, psychology, education, or closely related field preferred, or high school diploma or GED and approved professional development plan to obtain the associate's degree within 5 years of hire Experience 1 year of experience working with parents/caregivers and/or at-risk children Special Skills Possess behavioral management skills Possess good verbal and written communication skills Knowledge of child abuse and neglect issues, home visiting, parenting skills training, mental health and community resources. Ability to work independently and as a team member Ability to represent the agency in a professional manner within the community Ability to work in a collaborative manner with referral sources and team members Complete accurate paperwork in a timely manner Basic computer proficiency Bilingual/Biliterate in English & Spanish required Licensure, Registration, Certification Registered Agency Affiliated Counselor credential Willing to obtain endorsement through Center for Early Relational Health as Infant Family Associate or Early Childhood Family Associate, or higher Complete Mandated Reporter training upon hire First Aid/CPR, Bloodborne Pathogens and HIV training upon hire Valid driver's license and minimum required liability insurance for WA State. Must be deemed insurable as determined by Catholic Charities' liability insurance provider
Employment Conditional Upon:
Being cleared by criminal background check and fingerprinting when required
Work Schedule:
Monday-Friday, 8:00am - 5:00pm
Wage Range:
$22.00 - $25.50 per hour, depending experience
