Job Description

The position of a Provider Assistant is to provide support to the prescribers contracted and hired through CORE Health, the organization and the client(s) to ensure individuals are being provided the best possible care. The person in this position is responsible for supporting their assigned PMHNP for both virtual and in-office appointments that occur at the facility or remotely through telehealth. The Provider Assistant position is responsible for ensuring that clients are monitored for compliance, reminded of appointments, scheduled for follow-ups and appropriately roomed. This support includes schedule management and support for the PMHNP. Typical Duties Troubleshoot and ensure that telehealth platform (Blue Jeans) is working during in-office telehealth appointments; Collaborate with PMHNP to ensure smooth flow of communication prior to and during the appointment slots and throughout work day; Receive internal referrals for medication management services; Schedule clients for appointments with the PMHNP including follow-up appointments as directed by the PMHNP; Conduct appointment reminder phone calls; Manage PMHNP schedules ensuring appropriate availability and blocks are reflected; Review client chart to ensure compliance with program requirements, notify PMHNP when client is out of compliance and requesting new appointment, medication refills, etc.; Communicate refill requests to PMHNP; include details such as client compliance; communicate with Pharmacy as needed; Field phone calls from clients regarding Medication Management program; Coordinate communication between the PMHNP, the client's treatment team and/or client as needed; Assist clients with filling out paperwork including medication contracts and other forms and documentation; Scan and upload all documentation to the clients EHR (Avatar) including lab results, lab orders, third party our outside PCP and/or medication documentation, etc.; Ensure all communication follows established HIPPA and related regulations and guidelines including checks to ensure that current Releases of Information (ROIs) are on file; Manage PMHNP caseload assignments in the EHR system; Track client engagement in tracking systems; Update client contact, demographic and insurance information in the EHR as needed; Complete "No-show" and "Clinician Canceled" progress notes in the client medical record; Complete independent notes to document other services and communication as needed; Complies with HIPAA privacy and security regulations; Other duties as assigned by supervisor Qualifications Communicate effectively, orally and in writing, in both group and individual settings; Ability to work in a team based environment; Knowledgeable in HIPAA regulations; Implement quality process initiatives by utilizing tools to ensure customer satisfaction; Establishes and maintains effective interpersonal relationships with customers, peers, management, partners, community based organizations, private employers, and other state & public agencies; Performs computer applications & internet operations; Ability to sit at a computer for prolonged periods of time; Ability to lift up to 25 pounds at a time; Works in a fast paced environment demonstrating flexibility and ability to adapt to change; and, Recruits & engages customers into a system that provides a multitude of services to meet their needs.
