(AGRC) Why does the job exist? This position will provide direct case management to juvenile clients for the Juvenile Reintegration Centers to ensure clients are provided appropriate services while reintegrating into the community. Work with teams to ensure proper after care and placement are secured. How does it get done? This position will provide quality case management of all clients from arrival to discharge. Responsible for scheduling all client interviews for clients referred to the program. ensure thorough reviews of clients are conducted prior to suggesting placement. Ensure there are no issues with other referrals and current clients. Will establish and facilitate multi-disciplinary team meetings, ensuring the client needs are identified and addressed according to policy and procedures. Work with teams to solidify aftercare plans to include placement, treatment, education, vocational employment, and supervision components upon release and/or discharge. Ensure that all documentation are completed and relevant to the clients case. Ensure monthly plans are completed and followed by identified team members. Participate in Unit Managements meeting and provide suggestions for improvements in programming per the needs of the clients. Who are the customers? Juvenile Reintegration Clients, family and community partners Ideal Candidate Juvenile Reintegration Clients, family and community partners Minimum Qualification Associate's Degree in psychology, guidance and counseling, penology, sociology, social work, and/or criminology and two (2) years of experience in a correctional setting, military duty, classroom teaching or instructing in a rehabilitative environment, working in planned recreational activities, or active participation in community action programs, or for Correctional Officer Specialist positions, relevant work experience as it relates to the purpose of the position. Any combination of education from an accredited college or university in a related field or direct experience in this occupation totaling four (4) years may substitute for the required education and experience. Employment Requirements Completion of the hiring process is contingent upon successful completion of pre-employment screening process. This position is designated as safety sensitive. Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules and regulations in relation to alcohol and/or drug testing. Employment requires successful completion of post offer pre-employment physical. Pre-employment criminal background check is required and conditional pending results. Working Conditions Work performed in a correctional facility environment with exposure to potential risk from Juvenile actions. Incumbents must be willing to accept changes in geographic assignment and be willing to work shifts, overtime, holidays, weekends and call back for emergencies.
