Job Description

Join Encompass Health as a rehabilitation care professional supporting patients recovering from major illnesses and injuries. In this clinical role, you'll work within an interdisciplinary team to deliver patient-centered inpatient rehab services, assist with care planning, monitor patient progress, and educate patients and families. You'll collaborate closely with therapists, nurses, and physicians, maintain accurate clinical documentation, uphold safety and quality standards, and contribute to continuous improvement initiatives while growing your skills through training and advancement opportunities. Responsibilities Deliver patient-centered inpatient rehabilitation care to diverse patient populations Participate in interdisciplinary team meetings and coordinate care plans Monitor, document, and report patient progress using EMR systems Educate patients and families on recovery, safety, and home care needs Maintain compliance with clinical, safety, and infection-control standards Contribute to quality improvement and evidence-based practice initiatives Collaborate with therapists, nurses, and physicians to optimize outcomes Support orientation and mentoring of new team members when needed Required Skills Patient assessment Interdisciplinary care coordination Rehabilitation care planning Clinical documentation Patient and family education Vital signs monitoring Electronic medical records (EMR) Infection control and safety Outcome tracking and reporting Care quality improvement
