Job Description

A. POSITION SUMMARY
The Incident Management Investigator works closely with the Incident Management Investigator Specialist (IMIS) to conduct thorough investigations into situations which may violate a person's rights and/or protections in accordance with Office for People with Developmental Disabilities (OPWDD) and Justice Center regulations.
B. POSITION DUTIES
1. Works closely with the Incident Management Investigator Specialists. 2. Participates in weekly group and individual supervision as scheduled. 3. Initiates the investigative process by developing documentation "1st Request" for distribution to program teams. 4. Identifies potential witnesses and subjects of investigations for interview based on documentation review. 5. Conducts investigative interviews that address all aspects of the incident reported, as well as explores any systemic concerns. 6. Alerts the Director of Quality Assurance of any new incidents discovered during the investigative process and reports as needed and/or as directed by the Director of Quality Assurance. 7. Reviews investigative files for thoroughness and coordinates follow-up requests with IMIS team members. 8. Oversees the completion of

Final Reports by making any necessary revisions to drafts completed by the IMIS team and developing conclusions and recommendations in accordance with agency and regulatory standards addressing any and all issues identified through the investigation. 9. Completes any revisions following Incident Review Committee (IRC) Sub-Committee, OPWDD, and Justice Center (JC) reviews. 10. Testifies as needed for any grievance or JC Appeal hearings. 11. Escalate any obstacles to completing investigations to the Sr. Director of Investigations. 12. Other duties as assigned.
C. REQUIREMENTS
1. Bachelor's degree preferred in human services, criminal justice, psychology, social work, nursing or related field; equivalent relevant experience may be considered. 2. At least one year of experience in OPWDD services, incident management, investigations, quality assurance, compliance or a comparable human services setting. 3. Completion of agency approved and applicable Justice Center investigative training within a defined period after hire, with refresher training as required. 4.
Working knowledge of:
§ 14 NYCRR Parts 624 and 625; § applicable individual protections under Part 633; § the Protection of People with Special Needs Act; § mandated reporting requirements; and § agency incident-management procedures. 5. Ability to conduct objective, thorough, timely, and trauma-informed investigative interviews with individuals receiving services, witnesses, staff, and other relevant parties. 6. Ability to collect, preserve, review, analyze, and reconcile evidence, including records, statements, photographs, video, medical information, and electronic documentation. 7. Ability to prepare clear, factual, well-organized investigative reports that include credibility assessments, evidence-based conclusions, recommendations, and identification of systemic concerns. 8. Demonstrated ability to maintain confidentiality, impartiality, professional boundaries, and freedom from conflicts of interest. 9. Strong critical-thinking, analytical, writing, communication, organization, and time-management skills. 10. Ability to manage multiple investigations, meet regulatory and agency deadlines, respond promptly to communications, and escalate barriers or newly discovered incidents. 11. Proficiency with Microsoft Office, OneDrive, SharePoint, Teams, and applicable incident-reporting or investigative systems. 12. Ability to testify or present investigative findings during committee reviews, administrative proceedings, grievances, or appeals, when required. 13. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency's corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
