Lead MH/SA Case Manager -Outpatient Services Classification Lev-el:
9Summary:
This (FLSA Non-exempt) position has the responsibility to determine, provide, and/or coordinate available support services for the eligible individuals (children, adolescent, and/or adults) served by the Eastern Shore Community Services Board.General areas of responsibilities include:
Coordinating and providing case management services to facilitate the linkage of indi-vidual service needs with appropriate services within the agency, other human service agencies in the community, public and private hospitals, and with other interested per-sons as well as family members; Providing program oversight in conjunction with Case Management Supervisor to in-clude organizing and assigning new referrals, assessments, client contacts (direct and indirect), and field-based observation and support to ensure qualitative services are being delivered and client's needs are being met; and Completion of position-related documentation in accordance with applicable Medi-caid, licensure and agency regulations. In carrying out position responsibilities the incumbent reports to and receives supervision from the Case Management Supervisor. In carrying out position responsibilities, the incumbent performs in accordance with established ESCSB policies and procedures, demonstrates initiative, exercises sound judgement, maintains confidentiality of information, and sustains effective working relation-ships, and is able to work to productivity targets and exercise time management skills. The incum-bent also is required to possess demonstrable proficiency in the use of electronic heath records, be able to work both independently and collaboratively, and develop and implement well-organized services for the specified population.Essential Functions:
Conduct comprehensive assessments to determine case management needs; Maintain reduced case management caseload of Medicaid, waiver, and non-Medicaid indi-viduals; Identify appropriate community resources such as housing, transportation, financial assis-tance and all other necessary services in order to provide for individuals in need of such services; Provide outreach services to individuals who are at risk of or experiencing mental health/substance abuse diagnosis; Coordinate with appropriate staff, human service agencies, hospital, clinics, courts, proba-tion officers, and all other community resources in order to provide case management ser-vices for the chronically mentally disabled/at-risk individuals including assisting the indi-viduals and/or their family members to apply for benefits which they are eligible to receive; Conduct support/skill building groups to provide education for the individuals we serve; Link the individuals directly to services and support specified in the treatment plan; Provide and/or arrange transportation for individuals and/or their family member to access services; Assist with treatment planning; Make home visits as required; Provide discharge planning for individuals by linking service needs with appropriate re-sources within the agency, other human service agencies in the community, public and pri-vate hospitals, and for project funded or state hospital admitted individuals; Monitor service delivery to assure the adequacy and implementation of the treatment plan; Complete and maintain proper case documentation in accordance with licensure, Medicaid and agency regulations; As needed, provide supportive counseling on a limited basis, involving individuals' signifi-cant others (e.g. parents, friends, siblings) as needed to promote implementation of the ser-vices plan and optimal community adjustment; As needed, plan for and provide community-based crisis stabilization case management services for persons experiencing mental health, intellectual disabilities, and/or substance abuse emergencies; Attend meetings and in-service training activities (inter- and intra-agency staffings, confer-ences, seminars, etc.) related to position responsibilities; Provide day-to-day oversight and clinical support of the case management team; Conduct field-based observations as needed for the purpose of observing service delivery and identifying areas for program improvement; Assist in the development of program policies and procedures; Work closely with CSB Director, Coordinators, and Supervisors to identify and implement effective collaborations, best practices, and efficient and user-friendly client services; Coordinate with QI Director to ensure documentation meets Medicaid and licensing guide-lines; On an established schedule, meet with a representative of the Finance Department to review and monitor program revenues and expenses; Develop collaborative alliances with other agencies for the purpose of "mining" resources to benefit case management clients and/or programs; Study, understand, and stay current with licensing, Medicaid, and human rights, and other relevant regulations; Attend electronic health record meetings to integrate IT and EHR protocols with clinical practice; Orient, train, and support case managers in the use of the EHR and provide ongoing prob-lem-solving communications and collaboration regarding its use; Assist case managers in treatment plan development and progress note composition for all individuals; Attend all supervisory staff meetings related to services provision and administrative re-sponsibilities; Provide training and guidance during regularly scheduled staff meetings; and Perform other job-related duties as assigned by the Case Management Supervisor and/or the Director of Clinical Services and/or the Executive Director. Bachelor's degree in psychology, social work or related field required, minimum six (6) months position related experience required; Virginia licensure as a Qualified Mental Health Provider required; or any equivalent combination of education, training, and experience which provides the required knowledge and abilities.Required Knowledge, Skills and Abilities:
The incumbent should have position-related experience with the population served and/or with the coordination of service provision as well as supervisory experience and possess the following knowledge, skills, and abilities: The nature of serious mental illness in adults and serious emotional disturbance in children and adolescents; general knowledge of symptoms of addictive disorders and treatment of addictive disorders in adults and adolescents; treatment modalities and intervention tech-niques, such as behavior management, independent living skills training, supportive coun-seling, family education, crisis intervention, discharge planning and service coordination; different types of assessments, including functional assessment, and their uses in service planning; individuals' rights local community resources and service delivery systems; in-cluding support services (e.g. housing, financial, social welfare, dental, educational, voca-tional, legal/advocacy), eligibility criteria and intake processes, termination criteria and pro-cedures, and generic community resources (e.g. churches, clubs, self-help groups); types of mental health programs and services; types of substance abuse programs and services; ef-fective oral, written and interpersonal communication principles and techniques; required documentation for MH/SA case management services; the service planning process and de-velopment and implementation of treatment plans; the use of medications in the care and treatment of mental health/substance abuse population; position-related federal, state, and local regulations, practice and techniques of crisis intervention in-home delivery; the princi-ples and techniques of clinical staff supervision; and generally accepted safety procedures. Interviewing; observing, recording and reporting on an individual's functioning; identifying and documenting an individual's need for resources, services and other supports; using in-formation from assessments, evaluations, observation and interviews to develop service plans; identifying and documenting how services within the community, in the established service system and family supports can be utilized to promote achievement of the individu-al's personal habilitative/rehabilitation and life goals; formulating, writing and implementing individualized service plans to promote goal attainment for people with serious mental ill-ness, emotional disturbances, and substance abuse issues; negotiating with individuals and service providers, coordinating the provision of services by diverse public and private pro-viders, identifying community resources and organizations and coordinating resources and activities; using assessment tools (e.g. level of function scale, life profile scale, SA assess-ment tools); providing clinical supervision to direct care staff; implementing policies and procedures for service delivery; applying Medicaid, licensing, and human rights standards to day-to-day program operations; monitoring service delivery programs; working effective-ly with individuals with mental health, substance abuse, or co-occurring disorders; transpor-tation of individuals; communicating effectively; organization; completing required individ-ual documentation; modeling, teaching, and monitoring time management skills. Demonstrate a positive regard for individuals and their families (e.g. treating clients as indi-viduals, allowing risk taking, avoiding stereotypes of people with mental illness and sub-stance abuse, respecting individuals' and families' privacy, believing individuals are valua-ble members of society); be persistent and remain objective; work as a team member, main-taining effective inter- and intra- agency working relationships; communicate effectively, verbally and in writing; work independently under general supervision; establish and main-tain ongoing supportive relationships with the individuals served. Operate effective services within the parameters of agency policies and procedures and Medicaid, licensing, and hu-man rights regulations; develop and implement treatment plans; understand and work in an electronic health record; motivate, mobilize and support staff; exercise sound judgment; problem solve and handle emergencies; professionally and competently represent the agen-cy at meetings such as IEP and FAPT; maintain confidentiality; apply policies to program development; communicate policies to all affected parties; exercise, establish and maintain effective working relationships as necessitated by work assignments; problem-solve and handle emergent situations with sound judgment; and expand capacity by working with re-ferrals in community resources. The incumbent must have a valid driver's license and satisfactory driving record.Education and Experience:
