8:00 AM•4:00 PM | 40Hours/Week Rotation:
Rotating weekdays, rotating weekends, and occasional holidays Job Summary "Navitas Healthcare, LLC" is seeking an experienced Licensed Independent Clinical Social Worker (LICSW) for a travel assignment in an acute care hospital setting. The Social Worker will provide comprehensive psychosocial assessments, crisis intervention, discharge planning, and coordination of community resources to support patients and families throughout the continuum of care. The ideal candidate will possess strong clinical judgment, excellent communication skills, and the ability to thrive in a fast-paced healthcare environment. Key Responsibilities Conduct comprehensive psychosocial assessments for patients and families Evaluate emotional, behavioral health, substance use, and social factors impacting care Develop individualized discharge plans in collaboration with interdisciplinary care teams Provide crisis intervention, emotional support, and supportive counseling Assist patients and families in coping with illness, hospitalization, and life transitions Coordinate care with community agencies, behavioral health providers, and external resources Facilitate safe discharge planning, including long-term care or community placement when appropriate Advocate for patients navigating insurance, benefits, and social service programs Identify at-risk populations and initiate appropriate interventions Maintain accurate and timely documentation within electronic health record systems Participate in multidisciplinary rounds, case reviews, and quality improvement initiatives Ensure compliance with hospital policies, ethical standards, and social work regulations Serve as a liaison between patients, families, healthcare providers, and community organizations Provide social work consultation and support across multiple hospital departments as needed Required Qualifications Master's Degree in Social Work (MSW) required Active LICSW license in Massachusetts required Acute care hospital social work experience preferred CPI (Crisis Prevention Intervention) or MAB certification required within 1 month of hire and must be maintained annually Strong knowledge of psychosocial assessment, discharge planning, and crisis intervention Excellent communication, interpersonal, and organizational skills For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
